    Pre-Term Enrollment Request

    For students who have registered for a term which is not yet available through the Clearinghouse, please fill out the form below.

    Enrollment information will be sent to the Clearinghouse after the close of the first week of the term.

    If you are attaching a form to be completed, note the following:

    1. All fields requiring student or parent/guardian input must be completed and signed where appropriate.
    2. The LMU Student ID must be printed on the form.

    Incomplete forms, due to information or signature, will be returned unprocessed.
